"undefined entity"について、ふと思った程度の話なのですが、
使用している実体参照を、できる範囲で、対応が取れるUnicode文字に置き換えてみるというのはいかがでしょうか?

パラパラと眺めてみて、わざわざ実体参照にしなくてもいいんじゃないかなと思ったので。
